A little bit of both. Its not a remake in the sense of Resident Evil or Final Fantasy remakes. Those games had a huge budget, different stories, and feature no legacy aspects, which make them “full true reimagined games”.

MGS 3: Delta is a mid-tier budget game. New controls and gameplay improvements, but uses the old small PS2 map design with modern graphics placed on top. Enemy AI and boss fights should be extremely similar from the original. Voice acting from 2004 as well, with a near identical script.

In some ways this may be a good thing, I don’t want a Twin Snakes situation here, but in other ways, its close enough to the original game where it won’t feel like a new experience outside of the modern controls.

Some may claim its either a good or bad thing to make such a close graphical remake of the legendary game. But either way your stance may be, its not an ambitious game, that much is made clear.
