The sudden decision to move WA’s most difficult juvenile detainees to a maximum-security adult prison was met with shock by the upper ranks of the Department of Justice, a court has been told.

Melanie O’Connell, director of youth custodial operations within the women and young people division, testified at Cleveland Dodd’s inquest on Wednesday that she had long feared the worst before he was among 17 youths decamped from Banksia Hill to Unit 18 in late July 2022.
